Professional Responsibility Workload Report Forms

July 15, 2024

Completing a Professional Responsibility Workload Report Form is one of the best ways to meet your professional practice standards.

It allows you to document concerns and help to start discussions about promoting changes that benefits your patients, clients and residents. The form serves as protection for ONA members, shifting the accountability for resolution and legal liability back to their administrative leaders and the employer.

ONA central has a wealth of resources on the PRWRF!

Below are some SMGH-specific resources:

This is a copy of the Word doc version of the workload form. You can download this, fill this out on your computer or in printed format and send it to ONA (Stephanie and Patric), and to your manager.
Please e-mail the completed form to gro.a1732032823no@sy1732032823ramts1732032823pub551732032823lacol1732032823 and gro.a1732032823no@HG1732032823MSPV517320328235L1732032823
Please see the other files for the Workload Indicator checklist, and the tip sheet.
For workload forms that you have printed and written on, please either scan them to the e-mails or put the form in an envelope and slide it under the union office door addressed to ONA. The union office is on the 3rd floor near the chapel – room 3-434.
Please feel free to contact Patric and Stephanie at the emails above if you have any questions or concerns.

If your manager wants to speak with you about a workload form you can, and should, have a union rep present. Ask them to set up a meeting with ONA to discuss the concerns.

“Meet with your manager within 10 calendar days to discuss the PRWRF. A union rep can attend with you. The manager’s written response is due within 10 days of receipt of the PRWRF.

Meeting notes must be recorded, and any resolution achieved must be signed by all parties (consult with the Labour Relations Officer regarding the resolution).

Your union representative must be involved in all resolution discussions at the unit level.”
